Job Description: This position is responsible for the development, implementation, and facilitation of the Service Plan process for persons served, ensuring integration of service planning through the involvement of the individual, family, guardian, other service providers, agency staff, and others identified by the individual. 

Qualifications: Bachelor's Degree in a Human Service field of study required, and must meet all QMRP requirements.  Minimum of one year experience working with individuals with developmental disabilities and/or mental illness.  Effective organizational and communication skills, both written and oral.  Valid Illinois driver's license with good driving record.  Proficiency in the use of computers, software applications, and working knowledge of Microsoft Word and Outlook.
